Renaissance Care Link Clinic

The Renaissance Care Link Clinic provides patient-centered communication and disease-specific patient education, including:

  • Education classes to teach you the basics of your disease
  • Tools to monitor your health
  • The know-how of when to go to the ER, when to visit your physician, and tips for preventing a unnecessary ER visit


We also provide one-on-one sessions with a nurse and/or pharmacist to answer any questions you have about your disease, as well as monitor your vitals and medication regimen to provide specific recommendations for ways to improve your overall health.

As part of the Care Link Model of Care, we will provide:

  • Disease Self-Management
  • Nutrition Education
  • Medication Education and Therapy Management
  • Follow-up Phone Calls to see how you are doing and inquire if you need any kind of assistance or follow-up care
  • Home-Visits for those who are unable to visit the Renaissance Care Link Clinic
  • Community Resource Assistance, including counseling, vision screenings, and recommending a Primary Care Physician
