For Admitting purposes, patients should bring the following:

Health Insurance Card(s)

Photo Identification (e.g., valid driver’s license, passport)

Physician Orders

Prescription Medications: For inpatient and ambulatory procedures, patients should bring a list of their prescription medications.
How to register
Outpatient Registration
-
Patients may pre-register at least 24 hrs in advance of a scheduled ambulatory procedure. (Surgery, Cath Lab, Endoscopy, etc.)

Pre-Registrations are processed via the registration representative in the front lobby; after hours pre-registration are processed at the registration desk in the Emergency Department.

Patients undergoing ambulatory services outpatient surgery and or endoscopy should report to the registration front desk in the front lobby.
Patient Registration Hours
Monday – Friday 5:00 a.m. – 8:00 p.m.
Saturday 7:00 a.m. – 8:00 p.m.
When Patient Registration is closed, please report to the Emergency Department, for patient registration.
Inpatient Registration

All direct admit patients should report to the registration front desk in the front lobby.

After hours admission are processed at the registration desk in the Emergency Department.
