STEP 1: Attend an Informational Session at the Renaissance Bariatric and Metabolic Institute
The first step is to attend a FREE informational session at The Renaissance Bariatric and Metabolic Institute. Please call (956) 362-LOSE (5673) for a list of dates and times.
STEP 2: Complete Initial Patient Worksheet Questionnaire and Provide Insurance Information
The second step in your evaluation for weight lost surgery is to complete the prospective patient information worksheet and provide insurance information. As with all patient records the information that you provide is highly confidential. We would greatly appreciate your efforts in answering all questions to the best of your knowledge. If you can’t remember exact dates where needed, please provide approximate dates.
Do not leave any sections blank. If a section does not apply to your situation, please state “does not apply.” The information you provide is very important in helping us learn more about you, your general health and weight control issues. Our goal is to provide you the highest quality of care tailored to meet your health needs.
STEP 3: Insurance Coverage Verification for Weight Loss Surgery
The Renaissance BMI staff will contact your insurance company and verify whether your policy covers weight loss surgery!
Staff will ask your insurance carrier if the following procedures are covered at Renaissance BMI by your insurance plan. Renaissance BMI accepts most major insurance carriers including Medicaid and Medicare.
A VERY IMPORTANT NOTE: Most insurance companies require the following prior to surgery approval:
Medical documentation of a five-year weight history
Any actual documentation of diet drugs and medically supervised diets prescribed
Any commercial diet program records (Weight Watchers, Jenny Craig, etc).
Any exercise programs records (YMCA, YWCA, Gym memberships, etc.).
You will need to begin gathering these records NOW so that these are available when we communicate with your insurance company. It is NOT enough for you to simply list these items on the initial patient worksheet questionnaire. The insurance company will require official documentation.
STEP 4: Medical Qualification Appointment for Weight Loss Surgery with Surgeon
After you have attended an Informational Session, you may schedule an office visit with one of our surgeons to discuss a weight loss surgery plan. At this office appointment, the surgical staff will review your history and examine you briefly. You will have an opportunity to discuss surgical weight loss options with your surgeon and ask questions. At the conclusion of your visit a preoperative testing consultation plan and worksheet will be given to you. This plan includes a list of diagnostic test and consultations that you will be required to complete before proceeding with surgery.
STEP 5: Medical Weight Loss May Be Required
Your insurance carrier may require you to see a registered dietitian or physician for approximately 4 to 6 months consecutively before surgery. Please call your insurance in advance to confirm the required amount of appointments needed prior to medical weight loss surgery. These appointments are mandatory and must be scheduled on a monthly basis. If you miss your monthly appointment, your insurance carrier may require you to start all over again with your scheduled monthly appointments. Please keep in mind, that most insurances do not pay for the 4-6 visits prior to surgery. STEP 6: Medical Consultation and Assessments
You must complete all testing, assessments and consults that are ordered. Please note that all patients are required to have a nutritional and psychological evaluation done. The need for other consultations and evaluations will be determined by the history you provide, physical exam and the surgeon’s discussion with you. You will be provided with the names and phone numbers of consultants and testing areas so that you can make appointments that will be convenient for you.
STEP 7: Appointment for Pre-Surgery Bariatric Class
Once you have met the medical qualifications for surgery, you will need to schedule your Pre-Surgery Bariatric Class. Please call Renaissance BMI at (956) 362-LOSE (5673) to schedule your class. If this appointment cannot be kept, please notify us immediately.
STEP 8: Pre- Surgery Bariatric Class at the Renaissance Bariatric and Metabolic Institute
Please plan for an approximate 4-hour workshop program. We will measure your height and weight. You will be presented with information from our program staff regarding weight loss surgery risks. You will be given an opportunity to ask questions about benefits and what to expect.
Please come prepared for the workshop by:
1. Reviewing all material provided to you
2. Bringing the following items to assist you
A list of your questions
Reading and/ or distance glasses
We encourage all of you to bring a family member or a friend for support. Please do not bring your children. Again, please plan to be here approximately 4 hours while attending the workshop. At the end of the workshop, you will receive a certificate of attendance for your files.
STEP 9: Scheduling the Surgery Date with Surgeon
Once your insurance approval is obtained, you will set a surgery date with your surgeon
STEP 10: Pre-Op “Last Visit” at the Renaissance Bariatric and Metabolic Institute
The last visit will be scheduled 2 weeks prior to your surgery. During this visit you will be given a quiz that will be graded and any wrong answers will be reviewed with the nurse educator. A before picture will be taken (front and side shot) and data forms completed. The patient will be weighed and given any necessary Lab orders. The patient will be instructed on the 2 week pre-operative diet and any last minute questioned will be answered. Patients will receive post surgical follow up appointments and calendars with upcoming events (support group meetings).
STEP 11: The Surgery
In most cases you will be admitted to the hospital the morning of surgery. The actual time you will need to arrive will not be known until the day before surgery. Most surgical weight management patients are in the hospital for 2-3 days. Most patients return to work approximately 4 weeks after surgery or sooner.
STEP 12: Follow-up Visits at surgeon’s office and The Renaissance Bariatric and Metabolic Institute
