Bring a trusted friend or family member with you to help you better understand your diagnosis and treatment options. Plan for your first consultation to last an hour or more. Your physician will order a series of tests that will allow him or her to accurately stage and diagnose your cancer and create an accurate treatment plan. Becuase these tests may take several hours, we suggest you bring snacks and reading material. During this appointment, you'll also have an opportunity to meet with a financial counselor, nurse, and other members of the medical team who can inform you about our services at Cancer Center at Renaissance.
Forms
During your first visit you will be completing several forms, including a new patient intake and assessment form and a form requesting a list of all medications you're currently taking. Please arrive 20 minutes before your appointment time to complete the forms. If you need help completing the forms, let us know and we'll assist you.
Insurance
If recommended by your physician, a financial counselor will review your insurance information and explain which services will be covered. The financial counselor will then file claims for office visits and in-office procedures on your behalf. You'll receive monthly statements to help you keep track of services and treatments you receive at Cancer Center at Renaissance. If you don't have insurance, the financial counselor will discuss payment plan options and available assistance programs.
Medications
Bring any current prescription medications as well as an up-to-date list of all medications you're taking now, include over-the-counter ones and vitamins.
Medical records
Provide your medical team at Cancer Center at Renaissance with all previous lest results and any reports completed by your referring physician. If you referred yourself, we'll help you retrieve your previous medical records. The test results and reports may include X-ray, computed tomography (CT), or other diagnostic films: along with recent lab work; or pathology reports.
